Mail in your check payment:

Please make checks payable to your provider and not Pacific Harbor.
If possible please include the top portion of the statement you received.
Make sure to list your chart number and/or patient name in the memo field of your check.

Pay your bill by phone:

To make a payment by phone, please contact your provider.  We accept most major credit cards.
