Important Information about Insurance.   Below is a list of insurances some of our providers are contracted with.   Please note:   We do not take Medicaid or Apple Health Care insurances.

*Not all providers contract with all insurances. Please call our office to see which provider is accepting new patients and is contracted with your insurance.

Some health insurance companies require that you see a network provider to be eligible for full benefits.  In the event that your provider is not contracted with your insurance company, we will still work with and bill most insurances.  Our office staff can check with your insurance company. However, it is ultimately your responsibility to verify your plan and coverage for eligibility and benefits.  Some questions you might want to ask your insurance company include:

  • Whether the provider you have scheduled with or been referred to is a network provider?
  • Whether a prior authorization is or is not required?
  • What your out of pocket costs will be for the visit?
  • Whether the condition for which you are to be seen is a covered or excluded?
